All manufacturing organisations have equipment problems that plague their operations negatively impacting profits, and posing potential safety threats to their employees and the environment. Too often, organisations try to fix these problems quickly without ever finding what caused them in the first place, resulting in reoccurrence of the problems.

In this 3 day comprehensive training workshop the different RCFA methodologies will be explained with their benefits and disadvantages. The instructor will explain the RCFA process from initiating an analysis, writing a team charter, doing the analysis to implementing recommendations. The course contains practical examples and exercises.

This course differentiates itself from most other courses, because it focuses not only on the theory and practical exercises of doing a RCFA,  but equally important the course will provide practical information on how to implement a RCFA programme within your organisation. It highlights pitfalls that you can encounter and challenges faced during implementation of a RCFA programme.

The course contains useful information on developing a RCFA checklist, how to share lessons learned across your organisation, how to write a RCFA report and more.
